How do I put a form on my website to collect leads?

Build a form with your own questions, list the websites allowed to use it, then paste a snippet on your site. Every submission becomes a contact.

A lead form is a set of questions you embed on your own website. Anyone who fills it in becomes a contact in Peakify automatically. Building and editing lead forms is limited to your organization's admins.

Create a form

  1. Go to Settings, then Lead forms.
  2. Click Add a form, give it a name only your team will see, and press Create form. You land straight in the form's editor.
  3. If your account has a website address on file, we try to fill in the "Websites allowed to use this form" list for you automatically. If we could not read your website, or none is on file, that list starts empty and you add it yourself further down.

Add your questions

  1. Under Questions, pick a question type and press Add. You can add plain text, a paragraph, a dropdown with your own choices, or a checkbox, as many times as you like.
  2. First name, last name, full name, email address and phone number can each only be added once per form, because the answer maps onto that one field on the contact record. An email address or a phone number is what actually turns a submission into a contact.
  3. Use the up and down arrows to reorder questions, this is the order people see them in. Tick "People must answer this" for anything that should not be left blank.
  4. A form can have at most 40 questions.

Nothing is saved until you press Save changes. Reordering, adding and removing questions only changes your draft until then.

Ask permission to contact them

Under "Permission to contact them", write the wording shown next to a consent tick box, for example "Yes, you can email me about my enquiry." Leave it empty and no tick box is shown at all. If your form asks for a phone number, a second box for text-message permission appears, since permission to text is kept separate from permission to email. The exact wording each person saw is stored with their submission, so changing it later never rewrites what an earlier visitor agreed to.

List the websites allowed to use it

  1. Under "Websites allowed to use this form", list one web address per line, for example https://yourbusiness.com. Anything submitted from a website not on this list is turned away.
  2. Listing a website also covers everything underneath it, so yourbusiness.com covers www.yourbusiness.com too, you only need the one line. You can list at most 20 websites.
  3. This list is required before the form can be switched on, it is what stops a stranger who finds your form's snippet from using it on their own site.

Turn it on and add the snippet

  1. Tick "This form is switched on". If the checkbox will not tick, the reason is shown underneath, usually a missing website or no questions yet.
  2. Press Save changes.
  3. Copy the snippet under "Your snippet" and paste it into your website where you want the form to appear. If your website builder strips out the snippet and nothing shows up, use the fallback block shown under "Nothing showed up? Use this instead", it looks slightly different (a fixed height, and it shows your thank-you message on the same page instead of redirecting) but every submission is captured exactly the same way.

After someone submits it

Set a thank-you message, a page to send them to afterward, or both, under "After someone sends the form". Leave both blank and nothing happens on screen after they submit, though their answers are still saved.
